Privacy declaration

For proper treatment it is necessary that I, as the treating therapist, create a file. This is also a legal obligation imposed by the WGBO. A file contains notes on the state of health and data on the examinations and treatments carried out. The file also contains any additional information that is necessary for treatment and that I have requested, after explicit permission, from another healthcare provider, for example from the general practitioner.

I do my utmost to ensure the privacy of my clients. This means, among other things, that I handle personal and medical data with care and ensure that unauthorized persons do not have access to data. As a treating therapist, I have sole access to the data in my client's file. I have a legal duty of confidentiality (professional secrecy).

If necessary, the data from a file can also be used for the following purposes:

• To inform other care providers, for example when the therapy has been completed or when a referral is made to another practitioner. This only happens with explicit permission.

• For observation of my practice by a (pre-appointed) colleague, during my absence.

• For anonymized use in peer review.

• For administrative purposes: agenda management, and a small part of the data from a file is used for the financial administration, so that I or my administrator can draw up an invoice and file a declaration with the tax authorities. If I want to use data for another reason, I will always inform my clients first and ask for explicit permission.

This data in the client file will be kept for 15 years as required by the law on the treatment agreement.

Privacy on the invoice

The invoice contains the information requested by the health insurer, so that this bill can be claimed from the health insurer:
• Name, address and place of residence
• Date of birth
• Handle data
• Short description of the treatment (usually Psychosocial therapy)
• The cost of the treatment(s)
